Our in-house clinical pharmacists provide appointments with all on the day medication queries, carry out medication reviews and re-authorise repeat medications. They also support our pharmacy technicians and prescription clerks to ensure we respond to your medication queries and requests as promptly as we can. An important role of our pharmacists is to carry out safety audits to ensure medications are safely prescribed, to provide the best quality of care to all patients and to keep prescribing in line with national and local guidelines. Where a medication has been identified to be no longer safe for you to continue with, we will endeavour to communicate this to you via a letter, or a text or a phone call.

Our pharmacy technician’s process hospital discharge summaries and reconcile medication, process medication recommendations or changes to medication from clinic letters and ensure we have signed shared care agreements in place, where medications are prescribed based on a 'shared care agreement' between primary care and secondary care. They also help support our prescription clerks with on the day medication queries, support the GP practice with audits which require drug monitoring. Where a drug has been confirmed to be out of stock and has a long-term manufacturing issue, they will help find an alternative medication.

Prescription Clerks

 

Our prescription clerk’s main roles are to process repeat prescription requests and deal with prescription queries via the prescription line. They also work with care homes to process repeat and acute prescription requests and generate a list of patients who require a review with the GP during weekly ward rounds. If your prescription is due for re-authorisation, prescription clerks will liaise with a GP or pharmacist to have your repeat prescription reviewed and re-authorised. Please note you may receive a text message, which may involve blood monitoring, physical health monitoring or a questionnaire to be completed before your medication is re-authorised. If you have requested for a medication which is not on repeat, this will be sent to a GP or pharmacist to be reviewed or you may be asked to make an appointment.
