Online Services for Patients
Patients can book and cancel appointments, order repeat prescriptions and view details about their medications, allergies and adverse reactions on line. From April 2016 NHS England requires us to extend this service to enable patients to view detailed coded information held in their medical records on line.
In preparation for applying for Online Access there are some important things to consider. Please read the attached information leaflet Online Services Records Access Patient Information Leaflet “It’s Your Choice” Information is also available at NHS UK using this link.
If you wish to register please come to reception to collect the registration form or download it here. If you already have access to appointments and prescriptions online you will still need to complete the form and provide the necessary ID to access your records online.
Bring the completed form to the surgery with one form of photo ID and a second form of ID showing your current address (document must not be more than 3 months old) from the list below:
One forms of VALID photo ID from the following list:
- Photograph UK driving licence
- Student Card
- Disabled badge
- NHS Card
One form of VALID confirmation of address (not more than 3 months old)
- Bank Statement
- Credit card statement
- Building Society statement
- Mortgage statement
If you are unable to provide the required ID please contact the surgery to discus.
This service can be provided for parents/guardians of children under the age of 14 and parent/guardian will be required to complete the Proxy Access Consent Form. This service is only available for children up to the age of 14 when access will automatically be restricted by the IT system.
Access to this service can be provided to relatives or carers of our patients with the informed consent of that patient and if it is in their best interest. If you wish to apply for Proxy Access to a patients records please contact reception to discuss the process.
